School Based Health Center

Rocky Mountain Family Practice

Direct Primary Care model with a fixed, low-cost monthly membership that includes unlimited primary care access—no insurance required.

Elevated Community Health

Comprehensive care including medical, dental, vision, and behavioral health services.
